VK_MVK_moltenvk via loader

Hi,

In more recent MoltenVK versions (I am using 1.1.121) the MoltenVK extension VK_MVK_moltenvk and VK_EXTX_portability_subset and layer MoltenVK stopped appearing in the enumeration when using the loader.
If I load the MoltenVK.dylib instead of the vulkan.dylib, I get them back but the loose all the normal layers, like VK_LAYER_LUNARG_stadard_validation.

How do I load the and use the normal loaders, but still get access the MoltenVK bits?
